How Do I Add Custom Questions to My Review Form in Feedspace?

Every business has unique information it wants from reviewers. Feedspace lets you add up to three custom text questions to any review form so you can capture the specific details that matter most to you, beyond the standard name, email, and company fields.

What you can do

• Collect use-case details that make testimonials more compelling for prospects
• Gather industry-specific context that standard fields do not capture
• Identify patterns across reviewer responses to improve your product or service
• Build richer case studies using the additional detail reviewers provide

1. Open the user details tab

• Log in to Feedspace and go to Forms in the left sidebar.
• Click the pencil (edit) icon on the form card to open the form editor, which lands on the Capture Review tab.
• Click the User Details tab.

2. Add a custom field

Scroll to the bottom of the User Details tab and click Add Custom Field. A new custom field row appears.

3. Configure the custom field

For each custom field you can set:

• Label: The question text shown to the reviewer (e.g., “How did you hear about us?”).
• Placeholder: Hint text shown inside the input box.
• Type: Choose Text for a single-line input or Textarea for a longer multi-line response.
• Required: Toggle on to make the field mandatory before the reviewer can submit.

4. Add more fields (up to three)

Repeat the steps above to add up to three custom fields. The Add Custom Field button is hidden once you reach the maximum of three.

5. Remove a custom field

Click the delete icon next to any custom field to remove it from the form. Save the form to confirm the change.

6. Save the form

Click Save and Next. Your custom questions will now appear on the form when reviewers fill it out.

Common questions

What types of custom fields are available?

Custom fields support two types: Text (single line) and Textarea (multi-line). Both types accept plain text answers from reviewers.

Can I reorder custom fields?

Custom fields appear in the order they were added. To change the order, delete and re-add the fields in the desired sequence.

Where do custom field responses appear?

Responses are stored with the review and visible in the Testimonials section when you open an individual review.

How many custom questions can I add to a form?

You can add up to three custom fields on the User Details tab. Once you reach three, the Add Custom Field button no longer appears. Delete an existing custom field if you want to swap in a different question.

Can I make a custom field required?

Yes. Each custom field has a Required toggle. Turn it on and the reviewer cannot submit the form until they fill in that field, which is how you capture something mandatory like a last name.
