How Do I Choose Which Fields to Collect on My Feedspace Review Form?

The User Details step of your Feedspace review form collects additional information about the reviewer alongside their testimonial. You control exactly which fields appear by toggling each one on or off in the User Details tab. You can also mark any field as required so reviewers cannot skip it.

What You Can Do

• Show only the fields you need and hide the rest to keep the form short
• Make any field required so reviewers must fill it in before submitting
• Collect Name, Email, Job Role, Company Name, and Contact Number independently
• Combine standard fields with custom fields and a consent checkbox

1. Open the User Details Tab

• Log in to Feedspace and go to Forms in the left sidebar.
• Open the form and click the User Details tab.

2. Toggle Each Field On or Off

Under Setup Fields, toggle each field independently. The available standard fields are:

• Avatar/Photo: Profile photo upload (off by default). Reviewers can upload an image up to 15 MB.
• Collect Name: The reviewer’s full name or first name.
• Collect Email: The reviewer’s email address. Required if you want to use the reviewer email notification or automation features.
• Collect Job Role: The reviewer’s job title or position.
• Collect Company Name: The reviewer’s organisation or company.
• Collect Contact Number: The reviewer’s phone number (off by default).

3. Set Required Fields

Next to each enabled field is a Required checkbox. Tick it to make that field mandatory. Reviewers cannot submit the form without filling in required fields. Leave it unchecked to keep the field optional.

4. Save

Click Save & Next. The field changes take effect immediately for all new submissions.

Frequently Asked Questions

Which fields are enabled by default?

By default, Name, Email, Job Role, and Company Name are enabled. Avatar/Photo and Contact Number are off by default. None are marked required by default.

Can I add fields that are not in the standard list?

Yes. Click Add Custom Field below the standard fields to add up to three custom fields, each with its own label and placeholder text.

Does disabling the Email field affect anything else?

Yes. If Email is disabled, the reviewer email notification setting in the Settings tab is hidden, and automation emails cannot be sent to reviewers who did not provide an email. Enable Email if you plan to use either of these features.
